About ComplexCare Atlas
ComplexCare Atlas is the medical complexity front door of the Disability Atlas family, built and maintained by ConnectIDD.
What it is
ComplexCare Atlas brings together what families of children and adults with medical complexity need: care teams and programs in every state, plain-language guides, and tools families use every day.
Listings come from the Disability Atlas. State program lines come from HRSA and Family Voices. Medicaid details come from ConnectIDD research, with sources on each state page.
Who built it
ComplexCare Atlas is built and maintained by ConnectIDD. It grew out of conversations with parents and clinicians who live this every day.
